Diagnostic Sheets

Diagnostic sheets are available in two printable formats: HTML or .PDF. To use the .PDF file, you must have the Adobe Reader program installed on your computer. You can download the free program by clicking on the link at the bottom of this page. Both the HTML and the .PDF links open a new window. You may need to close that window to get back to this page.

Print the form. Complete all information and submit with a sample of the affected plant or fruit. Place the sample in a plastic bag and ship in a STURDY box. Mail at the beginning of the week to shorten the time in transit. Fresh material is required for an accurate diagnosis. Mail to your local extension office or to the Home & Garden Education Center, 1380 Storrs Rd., U-115, Storrs, CT 06269-4115.
